Insurance Plans Accepted
To determine if we participate with your insurance company you can either contact your insurance or call our offices.Cancellation Fees
We impose no-show fees (as stated below) that are applied to your account if you do not call in advance to cancel your appointment.Office visit requires 24 hours advance cancellation, fee is $25.
EMG/NCS requires 24 hours advance cancellation, fee is $50.
Cancelled check fee is $50.
Medication Refills
We do require 5 days notice to refill medications, so make sure you contact us in advance.
